About Coordinate my Care

Coordinate My Care (CMC) was established in May 2012 to address the need for patients to have integrated, coordinated and quality care.

We are an NHS clinical service and work to empower patients to have choices about the care they receive and to make those choices known to those who care for them. We look to continually link up the organisations and individuals that provide care for a patient including doctors, nurses, social care providers and emergency services including the ambulance service, NHS 111 and the out of hours GP service.


We work with GPs to support the needs and wishes of patients. GPs work together with their patients to create a personalised urgent care plan that is prepared and recorded on CMC to ensure all care providers can access this information all day, every day. We work closely with London Ambulance Service, the South East Coast Ambulance Service and NHS 111 to ensure patients receive the most appropriate care. We are developing partnerships across all of the NHS and the private sectors.


We know that Clinical Commissioning Groups, NHS Trusts, nursing homes, hospices, community providers, patients and their families all play an integral role in the successful coordination of care. As a team, we bring extensive clinical experience to drive the cultural and operational change required to deliver integrated, coordinated and quality care for patients.

Coordinate my Care

Patients & Carers

As a patient you likely have views about your care: what you would like to happen, where you want to receive care, and even things you would like to avoid.

Coordinate My Care is an NHS clinical service that supports you to record this information. A doctor or a nurse will talk to you about your health, illnesses and wishes and create a personalised urgent care plan that is available to help you. All these details will be entered into the Coordinate My Care system, which is an electronic record available to those who may need to care for you. It can be accessed by your GP, community nurses, hospital team, out-of-hours doctors, specialist nurses, London Ambulance Service and NHS 111 (NHS 111 is the unscheduled emergency telephone number that operates day and night). If you need help, the doctors and nurses caring for you either in the community or in the hospital will be able to access the clinical and personal information about you and know what care they should deliver to you.

Coordinate My Care gives you an opportunity for you to record your decisions and expressed wishes about your care. This information will be available to all legitimate professionals who are looking after you 24/7. The fact that everyone can see your care plan and wishes will help to ensure that any care you receive is in line with what you’ve decided. You have complete choice whether or not to participate.
